Modactive 200 mg is modafinil containing wakefulness promoting drug. It is used in treating excessive daytime sleepiness caused by narcolepsy, obstructive sleep apnea, or shift work disorder. It may work by:
- Dopamine reuptake inhibition: Weakly blocks dopamine transporter DAT may increase extracellular dopamine in brain, especially nucleus accumbens and prefrontal cortex. Less potent than methylphenidate.
- Orexin/hypocretin system: Activates orexin neurons in hypothalamus that promotes wakefulness. Narcolepsy patients are deficient in orexin.
- Histamine: Increases histamine release in tuberomammillary nucleus wake signal.
- GABA/Glutamate: Decreases GABA, increases glutamate in some brain areas cortical activation.
- No major sympathomimetic effect: Unlike amphetamines, minimal impact on peripheral norepinephrine. Less jitteriness, less blood pressure spike.
